Background:

Benign smooth muscle neoplasms can exhibit unusual growth patterns, including intravascular leiomyomatosis, benign metastasizing leiomyoma, and leiomyomatosis peritonealis disseminata.
Accurate classification of these rare entities is crucial for appropriate patient management.

Observation:

A unique case of perivascular leiomyomatosis is presented.
The tumor demonstrated multiple nodules of benign smooth muscle.
These nodules were associated with the outer surface of venous channels, specifically the medial muscle layer.

Findings:

The neoplastic nodules' location on the outer venous surface distinguishes this entity from intravascular leiomyomatosis.
Histological examination confirmed benign smooth muscle differentiation.
This presentation represents a novel growth pattern for benign smooth muscle neoplasms.
