  • Retrospective analysis of a prospective, observational cohort study.
  • Included 377 patients aged 65+ hospitalized for acute coronary syndrome, heart failure, or pneumonia.
  • Assessed understanding via post-discharge interviews, analyzing factors like jargon, team type, and notice before discharge.

  • Further research into provider-patient interactions within different teams is needed to improve hospital-to-community transitions.
