Background:

  • Pain is a common symptom in both functional dystonia and idiopathic cervical dystonia.

Purpose of the Study:

  • To investigate the sensory-discriminative and cognitive-emotional aspects of pain in patients with functional dystonia and idiopathic cervical dystonia.
  • To compare pain processing between these patient groups and healthy controls.

Main Methods:

  • Psychophysical testing of tactile and pain thresholds, and pain tolerance was conducted.
  • Electrical stimuli were applied to the index fingers and halluces of participants.
  • Pain threshold and tolerance were defined based on subjective sensory perception.

Main Results:

  • No significant differences in tactile or pain thresholds were observed across groups (functional dystonia, idiopathic cervical dystonia, controls).
  • Pain tolerance was significantly elevated in all tested body regions exclusively in patients with functional dystonia.
  • Continuous functional dystonia showed higher pain tolerance than paroxysmal functional dystonia and idiopathic cervical dystonia.

Conclusions:

  • Functional dystonia patients demonstrate a dissociation between sensory-discriminative and cognitive-emotional pain components, characterized by normal thresholds and heightened tolerance.
  • Altered pain processing in functional dystonia may be linked to aberrant connectivity between motor and limbic brain systems.
