Background:

  • Obstructive sleep apnea (OSA) is a common sleep disorder characterized by repeated upper airway collapse during sleep.
  • Current treatments for OSA include positive airway pressure (PAP) therapy and lifestyle modifications.
  • The role of lung volumes in OSA pathophysiology is not fully understood.

Observation:

  • A patient with sleep apnea syndrome experienced a reversal of their condition.
  • This was achieved by a 0.5 L increase in functional residual capacity (FRC).
  • The FRC increase was facilitated by constant negative extrathoracic pressure (NEP) using a poncho-type respirator.

Findings:

  • Pulmonary inflation via NEP led to a significant decrease in the apnea index and percent apnea time.
  • Sleep architecture showed marked improvement.
  • Desaturation duration was reduced across all sleep stages with NEP application.

Implications:

  • Lung volumes may play a crucial role in the pathophysiology of obstructive sleep apnea.
  • Increased lung volumes, potentially through enhanced upper airway patency, could be a novel therapeutic approach.
  • Further research is warranted to elucidate the precise mechanisms and explore NEP as a potential OSA treatment.
