Global Climate Change01:50

Global Climate Change

29.0K
Throughout its ~4.5 billion year history, the Earth has experienced periods of warming and cooling. However, the current drastic increase in global temperatures is well outside of the Earth’s cyclic norms, and evidence for human-caused global climate change is compelling. Paleoclimatology, the study of ancient climate conditions, provides ample evidence for human-caused global climate change by comparing recent conditions with those in the past.

Nuclear Fission

12.5K
Many heavier elements with smaller binding energies per nucleon can decompose into more stable elements that have intermediate mass numbers and larger binding energies per nucleon—that is, mass numbers and binding energies per nucleon that are closer to the “peak” of the binding energy graph near 56. Sometimes neutrons are also produced. This decomposition of a large nucleus into smaller pieces is called fission. The breaking is rather random with the formation of a large...

Empathy

10.1K
Some researchers suggest that altruism operates on empathy. Empathy is the capacity to understand another person’s perspective, to feel what he or she feels. An empathetic person makes an emotional connection with others and feels compelled to help (Batson, 1991). Empathy can be expressed in several ways, including cognitive, affective, and motor.

Summary

Severe secondary postpartum hemorrhage (PPH) is a rare but serious condition occurring up to 12 weeks after delivery. Emergency physicians must be aware of its distinct causes and management strategies compared to primary PPH.

Area of Science:

  • Obstetrics and Gynecology
  • Emergency Medicine

Background:

  • Secondary postpartum hemorrhage (PPH) is defined as PPH occurring 24 hours to 12 weeks after delivery.
  • While rare (0.23-3% of pregnancies), secondary PPH is more common after vaginal delivery than cesarean section.
  • Differential diagnoses and management strategies for secondary PPH differ significantly from primary PPH.

Observation:

  • A 27-year-old woman presented with severe vaginal bleeding one week post-cesarean section.
  • The patient required extensive medical and physical interventions, including uterine massage and multiple uterotonic agents, to control bleeding.
  • She necessitated a blood transfusion and experienced a significant drop in hemoglobin levels.

Findings:

  • Uterine atony, the primary cause of immediate PPH, is rarely the cause of secondary PPH.
  • This case highlights a severe instance of secondary PPH following cesarean delivery, necessitating urgent and comprehensive management.
  • The patient's presentation underscores the potential severity and complexity of managing delayed postpartum bleeding.

Implications:

  • Emergency physicians must maintain a high index of suspicion for secondary PPH, especially in postpartum patients presenting with bleeding.
  • Familiarity with the distinct etiologies and management protocols for secondary PPH is crucial for effective patient care.
  • Prompt recognition and appropriate intervention are vital for improving outcomes in patients experiencing severe secondary postpartum hemorrhage.
