Early recurrence of atrial tachyarrhythmia (ERAT) after pulmonary vein antrum isolation (PVI) predicts late recurrence. The blanking period should be limited to 23 days post-PVI to accurately define arrhythmia recurrence.
Area of Science:
- Cardiology
- Electrophysiology
- Medical Device Technology
Background:
- Atrial fibrillation recurrence after pulmonary vein antrum isolation (PVI) is common.
- The traditional 3-month blanking period may not accurately predict late recurrence (LR).
- Early recurrence of atrial tachyarrhythmia (ERAT) has been increasingly recognized as a predictor of LR.
Purpose of the Study:
- To determine the precise period after PVI when ERAT does not predict LR.
- To establish an evidence-based duration for the blanking period following PVI.
- To refine patient stratification for arrhythmia recurrence after PVI.
Main Methods:
- Analysis of baseline and follow-up data from 636 patients undergoing PVI (2010-2014).
- Monitoring of arrhythmia recurrence using ECG and Holter monitoring at set intervals.
- Receiver-operating characteristic (ROC) curve analysis to define the optimal blanking period.
Main Results:
- A significant association was found between ERAT within the first three months and LR (p=0.001).
- Patients with ERAT in the first, second, and third months post-PVI were 4.22, 9.03, and 19.43 times more likely to experience LR, respectively.
- ROC analysis identified 23 days post-PVI as the optimal cutoff for the blanking period (AUC=0.7, sensitivity=69.2%, specificity=61.2%).
Conclusions:
- The likelihood of LR increases with ERAT occurring after the first month post-PVI.
- The current 3-month blanking period is likely too long and may not be clinically optimal.
- A shortened blanking period of 23 days post-PVI is proposed for clinical practice and future research.
