Diffusion-weighted imaging (DWI) apparent diffusion coefficient (ADC) values correlate with electromyography (EMG) findings in myositis patients. This suggests both methods may reflect muscle fiber loss, aiding in myositis diagnosis.

  • MRI, particularly diffusion-weighted imaging (DWI), is increasingly used to assess muscle microstructural changes.
  • Electromyography (EMG) is a standard clinical tool for diagnosing myositis.

Purpose of the Study:

  • To investigate the relationship between apparent diffusion coefficient (ADC) values obtained from DWI and electromyography (EMG) findings in patients diagnosed with myositis.
  • To determine if ADC measurements can correlate with specific EMG parameters indicative of muscle damage.

Main Methods:

  • Seven patients with histopathologically confirmed myositis underwent 1.5-T MRI with DWI (b-values 0 and 1000 s/mm²).
  • Needle EMG was performed within three days of MRI, analyzing motor unit action potential (MUAP) amplitudes and durations, and spontaneous activity.
  • Spearman's correlation coefficient was used to analyze associations between ADC values (ADCmean, ADCmin, ADCmax) and EMG parameters.

  • Mean ADC values ranged from 1.28 to 1.73 × 10⁻³ mm²/s.
  • Significant positive correlations were found between ADCmean and MUAP duration (p=0.0279), and between ADCmin and MUAP duration (p=0.01).

Conclusions:

  • Apparent diffusion coefficient (ADC) values, specifically ADCmean and ADCmin, show strong positive correlations with motor unit action potential (MUAP) duration in myositis.
  • These findings suggest that DWI-derived ADC measurements may serve as a complementary imaging biomarker to EMG for assessing muscle fiber damage in myositis.
