Background:

  • Lymphomas are rare in the oral and maxillofacial region, with maxillary Non-Hodgkin's lymphoma (NHL) accounting for less than 1% of all NHL cases.
  • Diffuse large B-cell lymphoma (DLBCL) is an aggressive NHL subtype that can manifest in extranodal sites, including the maxilla.

Observation:

  • A 67-year-old woman presented with chronic pain and swelling, initially misdiagnosed as periodontal disease.
  • Imaging revealed extensive maxillary osteolysis, and biopsy confirmed DLBCL.

Findings:

  • Delayed diagnosis of maxillary DLBCL is common due to non-specific dental symptoms.
  • Computed tomography (CT) or cone beam computed tomography (CBCT) and immunohistochemistry are vital for diagnosis.

Implications:

  • Prompt diagnosis and combined R-CHOP chemotherapy with radiotherapy offer a favorable prognosis for maxillary DLBCL.
  • Increased awareness and advanced imaging are essential for early detection and management of oral lymphomas.
