Summary

Amiodarone may cause optic nerve damage through chronic neurotoxicity, leading to lipid deposits in large axons. This drug-induced lipidosis could explain some acute optic neuropathy cases.

Area of Science:

  • Ophthalmology
  • Neurology
  • Pharmacology

Background:

  • Amiodarone is linked to optic neuropathy, but the exact mechanism is unclear.
  • Patients on amiodarone are at higher risk for vasoocclusive events, complicating the direct drug effect assessment.

Observation:

  • Histopathological examination of retrobulbar optic nerve sections from an asymptomatic amiodarone user was performed.
  • The study focused on identifying potential drug-induced changes in the optic nerve.

Findings:

  • Lamellar inclusions were selectively observed within the large axons of the optic nerve.
  • These inclusions suggest a drug-induced lipidosis affecting the optic nerve.

Implications:

  • Amiodarone may exert a chronic neurotoxic effect on the optic nerve.
  • This drug-induced lipidosis mechanism could be responsible for previously reported acute optic neuropathies.
