Summary

A severe femoral shaft fracture in an 18-year-old man led to sudden hypoxemia and confusion. Prompt medical intervention, including mechanical ventilation and steroids, resulted in full recovery.

Background:

  • A healthy 18-year-old male sustained a left femoral shaft fracture from a high-velocity motorbike accident.
  • Initial computed tomography (CT) revealed no immediate abnormalities in the brain or chest.

Observation:

  • Within hours of admission, the patient developed acute mental confusion and severe hypoxemia.
  • Worsening tachypnea and abnormal arterial blood gas levels indicated significant respiratory compromise.

Findings:

  • A second contrast-enhanced chest CT and brain magnetic resonance (MR) imaging were performed.
  • The patient required intubation, mechanical ventilation, steroids, and noradrenaline for stabilization.

Implications:

  • Early recognition and aggressive management are crucial for patients with high-energy fractures presenting with neurological and respiratory compromise.
  • This case highlights the potential for delayed systemic complications following long bone fractures.
