Background:

Traumatic optic neuropathy (TON) often results in severe vision loss.
Intravenous corticosteroids are a common but not always effective treatment for TON.

Observation:

Two patients with profound vision loss due to TON did not improve with prior corticosteroid treatment.
Both patients underwent optic canal decompression using stereotactic navigation via an external transcaruncular approach.

Findings:

Following surgery, both patients experienced significant visual improvement.
Postoperative assessments showed enhanced Visual Evoked Potential (VEP) and improved optical coherence tomography (OCT) of the Retinal Nerve Fiber Layer (RNFL).

Implications:

Stereotactic navigation technology plays a crucial role in optic canal decompression for traumatic optic neuropathy.
This technique offers a viable surgical option for vision restoration in refractory TON cases.
