Summary

Tricuspid valvuloplasty combined with left-sided valve replacement offers a favorable prognosis for patients with severe pulmonary hypertension and tricuspid insufficiency. This surgical approach demonstrates high long-term survival and good patient condition post-operation.

Area of Science:

  • Cardiovascular Surgery
  • Cardiac Valve Disease
  • Pulmonary Hypertension

Background:

  • Tricuspid insufficiency accompanying severe left-sided valvular heart disease is linked to poor patient outcomes.
  • Severe pulmonary hypertension exacerbates the risks associated with tricuspid insufficiency.

Purpose of the Study:

  • To evaluate the efficacy and long-term results of tricuspid valvuloplasty performed concurrently with left-sided valve replacement in patients with severe pulmonary hypertension and tricuspid insufficiency.

Main Methods:

  • Twenty-two patients underwent tricuspid valvuloplasty after left-sided single or double valve replacement, post-cardiopulmonary bypass weaning.
  • Continuous right atrial pressure monitoring, specifically observing the V wave, was used to assess tricuspid annuloplasty effectiveness.

Related Experiment Videos

  • Successful annuloplasty was indicated by the obliteration of the right atrial pressure V wave peak.
  • Main Results:

    • Twenty-one out of twenty-two patients were long-term survivors.
    • Nineteen patients maintained good clinical condition at an average follow-up of 6.1 years (range: 1-12 years).

    Conclusions:

    • Tricuspid valvuloplasty is an effective adjunct to left-sided valve surgery in patients with severe pulmonary hypertension and tricuspid insufficiency.
    • This combined surgical strategy significantly improves long-term survival and functional status.
