Summary

Chronic hyperthyroidism rarely precedes anaplastic thyroid carcinoma. Progressive thyroid enlargement and acute worsening of hyperthyroidism warrant concern for malignancy, even with long-term toxic nodule management.

  • Despite long-term methimazole treatment for benign nodules, the goiter enlarged abruptly.

Findings:

  • Pathological examination confirmed undifferentiated anaplastic thyroid carcinoma.
  • The patient's condition rapidly deteriorated post-surgery.
  • This case highlights a rare instance of anaplastic transformation in a toxic multinodular goiter.

Implications:

  • Abrupt thyroid enlargement in patients with chronic hyperthyroidism requires prompt investigation for malignancy.
  • Worsening hyperthyroidism symptoms, irrespective of duration, can signal malignant transformation.
  • This case challenges the notion of chronic hyperthyroidism being solely protective against aggressive thyroid cancers.
