    Background:

    • Multiple myeloma (MM) is a common blood cancer where patient survival is impacted by extramedullary relapse (EM).
    • The mechanisms driving EM in MM are not well understood.
    • MicroRNAs (miRNAs) are key regulators of cellular processes and have been implicated in MM pathogenesis.

    Purpose of the Study:

    • To investigate the role of miRNAs in the development of extramedullary relapse (EM) in multiple myeloma (MM).
    • To identify differentially expressed miRNAs between MM patients and EM patients.

    Main Methods:

    • Next-generation sequencing was used to analyze miRNA expression in bone marrow samples.
    • Samples included 39 from MM patients at diagnosis and 9 from EM patients.
    • Bioinformatic analysis, including Bayesian normalization and Benjamini-Hochberg correction, was performed.

    Main Results:

    • A total of 2,278 miRNAs were sequenced, with 658 analyzed for expression.
    • Ten miRNAs were found to be statistically significantly differentially expressed between EM and MM patients (p < 0.0005).
    • Specific miRNAs identified include miR-26a-5p, miR-26b-5p, miR-30e-5p, and others.

    Conclusions:

    • This pilot study demonstrates distinct miRNA expression profiles in MM patients with extramedullary relapse.
    • The identified differentially expressed miRNAs may serve as potential biomarkers for EM in multiple myeloma.
    • Further validation using qPCR on a larger patient cohort is planned.
