    Background:

    • Early gastric cancer (EGC) detection rates were historically low.
    • Advances in endoscopic techniques have improved diagnostic capabilities.

    Purpose of the Study:

    • To compare the incidence and clinicopathologic features of EGC before and after the widespread adoption of endoscopy.
    • To evaluate the impact of endoscopic screening on EGC detection and patient outcomes.

    • Retrospective analysis of gastric resection specimens from two periods: 1967-1969 (pre-endoscopy) and 1977-1979 (post-endoscopy).

  • EGC comprised a significantly higher proportion of resected lesions in the post-endoscopic period (6/22) compared to the pre-endoscopic period (1/27).
  • Five of six EGC patients in the recent period survived without disease recurrence post-surgery, whereas the patient from the earlier period died postoperatively.
  • Endoscopic biopsy was diagnostic in all EGC cases, including those initially appearing as benign peptic ulcers on conventional imaging.
