    Summary

Chronic pancreatitis can mimic pancreatic cancer, presenting as a focal mass. Despite initial radiologic suspicion of malignancy, all patients were ultimately diagnosed with chronic pancreatitis.

    Area of Science:

    • Gastroenterology
    • Surgical Pathology
    • Diagnostic Imaging

    Background:

    • Chronic pancreatitis can manifest as a focal pancreatic mass.
    • This presentation is often radiologically indistinguishable from pancreatic carcinoma.
    • Accurate diagnosis is crucial for appropriate patient management.

    Purpose of the Study:

    • To evaluate the diagnostic accuracy of imaging and clinical presentation in differentiating focal chronic pancreatitis from pancreatic malignancy.
    • To analyze the outcomes of patients with suspected pancreatic masses due to chronic pancreatitis.

    Main Methods:

    • Retrospective review of radiologic studies in 21 patients with focal pancreatic masses.
    • Analysis of surgical findings (laparotomy, pancreatectomy, biopsy) and percutaneous biopsy results.
    • Correlation of initial diagnoses with final histologic confirmation of chronic relapsing pancreatitis.

    Main Results:

    • Radiologic studies suggested carcinoma in 16 patients and islet cell tumors in 5.
    • Seventeen patients underwent laparotomy due to a palpable mass.
    • Histologic examination confirmed chronic relapsing pancreatitis in all 21 patients.

    Conclusions:

    • Focal chronic pancreatitis can present as a mass mimicking pancreatic cancer.
    • Initial radiologic findings may be misleading.
    • Histologic confirmation is essential for definitive diagnosis in suspected pancreatic masses.
