Germany's colorectal cancer quality project improved diagnostic compliance but not surgical outcomes. Further training investment is needed for better surgical results in this national quality assurance initiative.
Area of Science:
- Oncology
- Public Health
- Health Services Research
Background:
- Nationwide quality programs in colorectal cancer (CRC) are associated with improved patient outcomes.
- Germany has a voluntary, self-organized, and self-financed observational quality assurance project for CRC.
Purpose of the Study:
- To evaluate if Germany's nationwide CRC quality assurance project leads to improved patient outcomes.
- To assess the impact of the quality assurance project on treatment standards and patient results.
Main Methods:
- Analysis of data from the German Quality Assurance in Colorectal Cancer Project (2000-2012), including 11,597 patients from 41 hospitals.
- Parameters analyzed included patient demographics, tumor staging, use of preoperative imaging (CT, MRI), and surgical outcomes (CRM status, complications, mortality).
Main Results:
- Increased use of preoperative CT (67.2% to 88.8%) and MRI (5.0% to 35.2%) was observed.
- A significant decrease in intraoperative (7.9% to 5.3%) and postoperative surgical complications (28.0% to 18.6%) occurred.
- No significant changes were noted in histopathological stage, anastomotic leakage, or in-hospital mortality.
Conclusions:
- Participation in the quality assurance project enhanced adherence to diagnostic standards.
- Improvements in surgical outcomes necessitate additional investment in healthcare professional training.
