  • Portal vein thrombosis is an infrequent but severe potential complication.

Observation:

  • A 21-year-old woman underwent an uncomplicated laparoscopic sleeve gastrectomy.
  • Postoperatively, she developed thrombosis in the portal, splenic, and right common iliac veins.

Findings:

  • The extensive venous thrombosis led to multifocal brain abscesses.
  • Septic emboli, presumed to be from Fusobacterium, are suspected as the cause of the brain abscesses.

Implications:

  • This case underscores the rare but critical risk of extensive venous thrombosis following sleeve gastrectomy.
  • Early recognition and management of portal vein thrombosis are crucial in bariatric surgery patients.
  • Further research may be needed to elucidate the mechanisms and preventative strategies for this rare complication.
