Cardiomyopathy VII: Pre and Post Operative Nursing Management

362
Patients with hypertrophic cardiomyopathy (HCM) and left ventricular outflow tract (LVOT) obstruction who remain symptomatic despite optimal medical therapy may undergo a septal myectomy (Morrow procedure). This procedure involves excising a portion of the hypertrophied septum below the aortic valve using a heart-lung machine to improve blood flow through the LVOT. Effective preoperative and postoperative nursing management ensures successful patient outcomes, minimizes complications, and...

Pain01:20

Pain

1.4K
Pain serves as a critical warning signal that alerts the body to potential or actual harm. When mechanical pressure on the skin is intense, such as from a sharp pinch, the sensation transitions from touch to pain. Similarly, extreme temperatures, like a hot pot handle, convert the sensation of heat into pain. Pain can also result from overstimulation of other senses, such as blinding light, loud noise, or the intense heat from habañero peppers. This ability to sense pain is essential for...

Operational Amplifiers01:17

Operational Amplifiers

2.0K
The operational amplifier, often referred to as an op-amp, is a multifaceted building block of a circuit. This electronic component functions like a voltage-controlled voltage source and can also be used to create a voltage- or current-controlled current source. The design of an operational amplifier enables it to execute mathematical operations when external components like resistors and capacitors are linked to its terminals. An op-amp has the capacity to sum signals, amplify a signal,...

Real Number Operations01:27

Real Number Operations

339
The concept of real numbers includes all the values that can be represented on a continuous number line. The system began with basic counting values used for enumeration. It later expanded to include values that represent the absence of quantity and opposites of the counting values. When situations required expressing parts of a whole or dividing quantities evenly, values capable of representing such proportions were developed. When written using decimal notation, these values can end or repeat...

Vector Operations01:20

Vector Operations

2.3K
Vectors are physical quantities that have both magnitude and direction. The vector operations include addition, subtraction, and scalar multiplication.
A vector multiplied by a scalar value is called scalar multiplication. The result obtained is a new vector with a different magnitude. If the scalar is positive, the direction of the vector remains the same, but if it is negative, the direction of the vector is reversed. For example, the product of the mass and velocity yields the momentum.

Operant Conditioning01:21

Operant Conditioning

2.9K
Operant conditioning, a key concept in behavioral psychology, involves using reinforcement and punishment to alter the likelihood of a behavior being repeated. B.F. introduced this type of conditioning. Skinner focused on voluntary behaviors and the consequences that follow them, influencing whether these behaviors will be strengthened or diminished.
Reinforcement in operant conditioning can be positive or negative, both of which serve to increase the likelihood of a behavior. Positive...

Summary

Peri-tubal bupivacaine infiltration effectively reduced pain after percutaneous nephrolithotomy. This study found lower pain scores and reduced analgesic use in patients receiving bupivacaine compared to saline.

Area of Science:

  • Urology
  • Anesthesiology
  • Surgical Pain Management

Background:

  • Percutaneous nephrolithotomy (PCNL) is a common procedure for kidney stones.
  • Post-operative pain is a significant concern following PCNL.
  • Effective pain management strategies are crucial for patient recovery and satisfaction.

Purpose of the Study:

  • To evaluate the efficacy of peri-tubal local anesthetic infiltration in managing post-operative pain after PCNL.
  • To compare pain levels and analgesic consumption between patients receiving bupivacaine and those receiving normal saline.

Main Methods:

  • A case-control study involving 68 patients undergoing PCNL.
  • Patients were randomized into two equal groups: cases (bupivacaine infiltration) and controls (normal saline infiltration).
  • Post-operative pain was assessed using the Visual Analogue Scale (VAS).

Main Results:

  • The bupivacaine group (cases) showed significantly lower post-operative pain scores at 6 and 12 hours compared to the control group (p<0.05).
  • Total analgesic consumption and demand for pain medication were significantly reduced in the bupivacaine group (p<0.05).

Conclusions:

  • Peri-tubal infiltration with 0.25% bupivacaine is an effective method for reducing post-operative pain after PCNL.
  • Bupivacaine infiltration offers a safe and efficient approach to enhance pain relief and decrease analgesic requirements in PCNL patients.
