    • Primary hypertrophy of the pylorus is a rare condition in adults.
    • It can present with symptoms mimicking gastric outlet obstruction.

    Observation:

    • A 52-year-old patient presented with symptoms suggestive of infiltrative pyloric cancer.
    • Diagnostic methods included videothoracoscopy, fluoroscopy, electrogastrogram, and fluorescence spectroscopy.

    Findings:

    • Histological examination post-gastric resection revealed hypertrophy of circular and longitudinal muscle fibers in the pylorus.
    • Inflammatory changes were also noted.

    Implications:

    • Primary pyloric hypertrophy should be considered in the differential diagnosis of suspected infiltrative pyloric cancer in adults.
    • Accurate diagnosis is crucial for appropriate patient management.
