Total internal reflection fluorescence microscopy or TIRF is an advanced microscopic technique used to visualize fluorophores in samples close to a solid surface with a higher refractive index, such as a glass coverslip. Summary

Preoperative narcotic use is the primary risk factor for prolonged use after total hip arthroplasty (THA). Both preoperative and prolonged postoperative narcotic use increase complication risks, including infection and revision surgery.

Purpose of the Study:

  • To identify risk factors for prolonged narcotic use following THA.
  • To evaluate the impact of preoperative and prolonged postoperative narcotic use on THA complications.

Main Methods:

  • Analysis of a national database of primary THA patients (2007-2015).
  • Identification of preoperative (POU) and prolonged postoperative narcotic users (PPU).
  • Multivariable logistic regression to determine risk factors and complication associations.

Main Results:

  • 18,740 (33.8%) patients used preoperative narcotics (POU), and 14,996 (27.1%) were prolonged postoperative users (PPU).
  • Preoperative narcotic use was the strongest predictor of prolonged postoperative use.
  • Both POU and PPU were significantly associated with increased postoperative complications.

Conclusions:

  • Preoperative narcotic use is the most significant patient-specific risk factor for prolonged postoperative narcotic use after THA.
  • Patients with POU and PPU face higher risks of postoperative infection and revision surgery.
