    Background:

    • Amiodarone is a widely used antiarrhythmic drug.
    • Long-term amiodarone therapy is associated with various toxicities.
    • Peripheral neuropathy is a potential adverse effect.

    Observation:

    • Three patients presented with peripheral neuropathy.
    • Neuropathy onset occurred after more than 18 months of amiodarone treatment.
    • All patients exhibited elevated serum amiodarone and desethylamiodarone levels.

    Findings:

    • Nerve biopsy in one patient revealed amiodarone and metabolite concentrations 80-fold higher than serum levels.
    • High drug concentrations correlate with the development of peripheral neuropathy.
    • Sustained high-dose amiodarone therapy is linked to neurotoxicity.

    Implications:

    • Peripheral neuropathy is a significant complication of prolonged amiodarone use.
    • Monitoring drug levels may be important in patients on long-term amiodarone.
    • Clinicians should be vigilant for neurological symptoms in patients receiving amiodarone.
