Background:

Colloid cysts typically arise in the anterior third ventricle near the foramina of Monro.
Extension into the lateral ventricle is uncommon but possible.

Observation:

A rare case of three intraventricular colloid cysts is presented: two in the third ventricle and one in the lateral ventricle.
The patient had a history of hydrocephalus treated with a ventriculoperitoneal shunt.
Magnetic resonance imaging revealed distinct signal characteristics for each cyst.

Findings:

Endoscopic microsurgery successfully removed all three cysts.
The lateral ventricular cyst's removal facilitated access to the third ventricle cysts due to an enlarged foramen of Monro.
Postoperative imaging confirmed complete removal with no recurrence or hydrocephalus after two years.

Implications:

Colloid cysts can grow significantly and extend into the lateral ventricle, particularly in patients with ventriculoperitoneal shunts.
Detailed pathoanatomic understanding is crucial for surgical planning and approach selection in complex colloid cyst cases.
