Background:

  • Colloid cysts typically arise in the anterior third ventricle near the foramina of Monro.
  • Extension into the lateral ventricle is uncommon but possible.

Observation:

  • A rare case of three intraventricular colloid cysts is presented: two in the third ventricle and one in the lateral ventricle.
  • The patient had a history of hydrocephalus treated with a ventriculoperitoneal shunt.
  • Magnetic resonance imaging revealed distinct signal characteristics for each cyst.

Findings:

  • Endoscopic microsurgery successfully removed all three cysts.
  • The lateral ventricular cyst's removal facilitated access to the third ventricle cysts due to an enlarged foramen of Monro.
  • Postoperative imaging confirmed complete removal with no recurrence or hydrocephalus after two years.

Implications:

  • Colloid cysts can grow significantly and extend into the lateral ventricle, particularly in patients with ventriculoperitoneal shunts.
  • Detailed pathoanatomic understanding is crucial for surgical planning and approach selection in complex colloid cyst cases.
