    Background:

    Purpose of the Study:

    • To compare antiretroviral adherence and viral load changes in HIV-infected outpatients enrolled versus not enrolled in medication synchronization.
    • To evaluate the impact of medication synchronization on antiretroviral therapy (ART) adherence and viral suppression.

    Main Methods:

    • A multicenter, retrospective pilot cohort study included 58 insured, HIV-infected adult outpatients.
    • Data on proportion of days covered (PDC), viral load, and adherence were collected and compared between medication synchronization enrollees and a control group.
    • Study endpoints were analyzed retrospectively after a 3-month period.

    Main Results:

    • Patients in medication synchronization had significantly higher PDC (96% ± 9%) compared to controls (71% ± 27%).
    • Medication synchronization was associated with increased ART adherence (OR 10.67).
    • While viral load became undetectable in a higher percentage of the synchronization group by study completion (83.3% vs. 64.7%), no statistically significant change in viral load was observed between groups.

    Conclusions:

    • Medication synchronization programs are associated with improved ART adherence in insured, HIV-infected adult outpatients.
    • Further research is necessary to fully ascertain the clinical impact of medication synchronization on patient outcomes.
