Summary

Thyroid ultrasounds can rarely cause transient hyperthyroidism in patients with large goiters. This condition resolved spontaneously without medication, suggesting hormone release from compressed nodules.

  • Transient hyperthyroidism is known after thyroid trauma or surgery.
  • Previous reports link it to parathyroidectomy, neck trauma, strangulation, and palpation.
  • No cases have been linked to diagnostic thyroid ultrasound.

Observation:

  • A 58-year-old woman with a large nontoxic multinodular goiter underwent a thyroid ultrasound.
  • She developed transient hyperthyroidism post-procedure.
  • No anti-thyroid medications were administered.

Findings:

  • Thyroid hormone levels (free T3, free T4) normalized within two weeks.
  • Thyroid Stimulating Hormone (TSH) and other thyroid tests returned to normal range by 11 weeks.
  • The patient's hyperthyroidism resolved spontaneously.

Implications:

  • This is the first reported case of hyperthyroidism induced by thyroid ultrasonography.
  • The mechanism may involve pressure from the ultrasound probe compressing nodules and releasing stored thyroid hormone.
  • Highlights a rare potential complication of thyroid ultrasound in specific patient populations.
