    Area of Science:

    • Gastroenterology
    • Oncology
    • Colorectal Surgery

    Background:

    • Chronic ulcerative colitis (CUC) is an inflammatory bowel disease associated with an increased risk of colorectal cancer.
    • Epithelial dysplasia is a precancerous condition that can precede the development of invasive carcinoma in CUC patients.

    Observation:

    • A long-standing case of chronic ulcerative colitis (17 years) presented with epithelial dysplasia and subsequent colonic carcinoma.
    • During surgical exploration for the colonic carcinoma, Crohn's disease of the terminal ileum was incidentally identified.

    Findings:

    • The resected terminal ileum exhibited macroscopic and histological features definitively characteristic of Crohn's disease.
    • This finding suggests the potential for co-occurrence of distinct inflammatory bowel diseases in the same patient.

    Implications:

    • Highlights the importance of thorough intraoperative examination, even when a primary diagnosis is apparent.
    • Underscores the complex interplay and potential co-existence of different gastrointestinal pathologies.
    • May inform diagnostic considerations in patients with long-standing inflammatory bowel disease and associated complications.
