    Background:

    • Traumatically induced central brain hemorrhages are uncommon, representing a small percentage of craniocerebral trauma cases.
    • These hemorrhages frequently occur alongside other severe cranial and intracranial injuries.

    Purpose of the Study:

    • To investigate the incidence, location, characteristics, and prognosis of traumatic central brain hemorrhages.
    • To differentiate between primary and secondary traumatic hemorrhages and assess outcomes.

    Main Methods:

    • Retrospective analysis of 3598 patients undergoing computed tomography (CT) for craniocerebral trauma between 1976 and 1984.
    • Identification and characterization of central brain lesions, including location, size, and association with other injuries.

    Main Results:

    • Central lesions were found in approximately 3% of trauma patients, predominantly in the basal ganglia, often extending to form large intracerebral hematomas.
    • The right hemisphere was more frequently and severely affected than the left. The thalamus was rarely the origin of large hematomas.
    • A 42% lethality rate was observed, strongly correlated with the severity of associated brain lesions. Isolated central hemorrhages showed a favorable prognosis.

    Conclusions:

    • Traumatic central brain hemorrhages, particularly isolated ones, may have a better prognosis than previously assumed.
    • Smaller central lesions appear to be more frequent and survivable.
    • The basal ganglia are the most common site, with a higher incidence in the right hemisphere.
