Background:

  • Kappa opioids are implicated in stress responses in animal models.
  • The role of kappa opioid receptors (KOR) in human depression and stress is not well understood.

Purpose of the Study:

  • To quantify in vivo KOR binding in individuals with major depressive disorder (MDD) using positron emission tomography (PET).
  • To investigate the relationship between KOR binding and diagnosis, life stress, and stress reactivity.

Main Methods:

  • PET imaging with [¹¹C]GR103545 radiotracer was used to measure KOR binding (total volume of distribution, VT).
  • 10 MDD participants and 13 healthy volunteers were scanned.
  • Analyses examined KOR VT in relation to diagnosis, childhood trauma, recent life stress, and cortisol levels during a modified Trier Social Stress Test (mTSST).

Main Results:

  • No significant differences in KOR VT were found between MDD participants and healthy volunteers in predefined regions of interest.
  • KOR VT was not significantly correlated with childhood adversity or recent life stress.
  • A trend-level inverse correlation was observed between KOR VT and cortisol response during the mTSST (p=0.081).

Conclusions:

  • In this pilot study, in vivo KOR binding does not differentiate MDD from healthy volunteers.
  • Future research should investigate KOR in specific MDD subgroups, such as those with co-occurring substance use disorders or psychotic features.
