Low-dose deoxycoformycin (dCF) shows antineoplastic potential in advanced lymphoid malignancy. This adenosine deaminase inhibitor demonstrated objective responses and clinical improvement without prohibitive toxicity in chronic lymphocytic leukemia (CLL) patients.

    Area of Science:

    • Oncology
    • Pharmacology

    Background:

    • Deoxycoformycin (dCF) is a potent inhibitor of adenosine deaminase (ADA).
    • Both normal and malignant B lymphocytes exhibit low ADA activity.
    • Low doses of dCF can profoundly inhibit ADA in chronic lymphocytic leukemia (CLL) patients' peripheral blood.

    Purpose of the Study:

    • To explore the antineoplastic potential of deoxycoformycin (dCF) in patients with advanced lymphoid malignancy.
    • To assess the safety and efficacy of low-dose dCF in this patient population.

    Main Methods:

    • Administered low doses of dCF (4 mg/m2) to 28 patients with advanced lymphoid malignancy.
    • Monitored objective responses and clinical improvement.
    • Assessed pretreatment adenosine deaminase (ADA) activity in responding and non-responding patients.

    Related Experiment Videos

    Main Results:

    • Five out of 28 patients achieved an objective response.
    • Four additional patients experienced clinical improvement.
    • No significant difference in pretreatment ADA activity was observed between responders and non-responders.
    • Low-dose dCF (4 mg/m2) was not associated with prohibitive toxicity.

    Conclusions:

    • Low-dose deoxycoformycin (dCF) demonstrates antineoplastic activity in advanced lymphoid malignancy, including chronic lymphocytic leukemia (CLL).
    • dCF may offer a treatment option for patients who have failed standard alkylating agents, suggesting a lack of cross-resistance.
    • Further investigation of low-dose dCF in advanced malignancies is warranted.
