Schemas

12.4K
A schema is a mental construct consisting of a cluster or collection of related concepts (Bartlett, 1932). There are many different types of schemata, and they all have one thing in common: schemata are a method of organizing information that allows the brain to work more efficiently. When a schema is activated, the brain makes immediate assumptions about the person or object being observed.

    Area of Science:

    • Psychiatry and Pharmacology
    • Neuroscience and Mental Health

    Background:

    • Significant unmet needs exist in psychotropic drug efficacy, tolerability, and onset of action.
    • Despite the high burden of mental disorders, few novel psychotropic medications have recently been approved in Germany.
    • Economic factors and assessments of additional benefit significantly influence market availability.

    Purpose of the Study:

    • To review recent developments in psychotropic drug introductions and withdrawals in Germany and the USA.
    • To highlight advancements in the treatment of schizophrenia, ADHD, depression, alcohol consumption, and obesity.
    • To discuss challenges and the urgent need for continued psychotropic drug development.

    Main Methods:

    • Review of recently introduced and withdrawn psychotropic medications in key markets (Germany, USA).
    • Analysis of specific drug classes, including antidepressants, antipsychotics, and agents for substance use and metabolic disorders.
    • Consideration of novel formulations and administration routes (e.g., inhalative, long-acting depots).

    Main Results:

    • Lurasidone and vortioxetine withdrawn in Germany; nalmefene, guanfacine, milnacipran, and liraglutide introduced.
    • New atypical antipsychotics (cariprazine, brexpiprazole) released in the USA; long-acting antipsychotics approved in Europe.
    • Loxapine available for agitation; quetiapine recommended for treatment-resistant depression; ketamine and botulinum toxin under investigation.

    Conclusions:

    • The psychotropic drug development pipeline faces significant economic and regulatory hurdles, alongside negative public perception.
    • Recent innovations include long-acting antipsychotics, novel antidepressants, and treatments for specific conditions like ADHD and alcohol dependence.
    • Urgent constructive efforts are required to prevent stagnation in the development of essential psychotropic medications.
