    • Retrospective analysis of 87 patients undergoing focused parathyroidectomy (2011-2015).
    • Patients grouped based on time to >50% ioPTH drop (≤15 minutes vs. >15 minutes).
    • Comparison of preoperative PTH, calcium, GFR, age, and adenoma weight between groups.

    Main Results:

    • Lower preoperative and preincision PTH levels were significantly associated with requiring >15 minutes for a >50% ioPTH drop.

    Conclusions:

    • Mildly elevated preoperative PTH may predict a slower PTH decline post-excision.
    • Further research is needed to determine optimal thresholds for intraoperative decision-making, such as proceeding with four-gland exploration, based on PTH drop kinetics.
