Urethral syndrome (abacterial cystitis)--search for a pathogen.

    Summary

Recurrent urethral syndrome in women is rarely caused by bladder infections. Investigations suggest inflammation, not microbes, is the likely cause of abacterial cystitis.

    Area of Science:

    • Urology
    • Microbiology
    • Pathology

    Background:

    • Recurrent urethral syndrome, also known as abacterial cystitis, affects numerous women.
    • The etiology of this condition remains largely enigmatic, with limited understanding of potential causes.

    Purpose of the Study:

    • To investigate the microbiological, cytological, and histological factors associated with recurrent urethral syndrome.
    • To identify potential etiological agents or underlying pathological processes in affected women.

    Main Methods:

    • Extensive microbiological analysis of bladder aspirates and voided urine.
    • Cytological examination of urethral samples.
    • Histological examination of bladder biopsies.
    • Cystoscopic evaluation.

    Related Experiment Videos

    Main Results:

    • Microbiological investigations identified a potential cause in only 4 out of 31 patients (Streptococcus species, wall-deficient Streptococcus, Lactobacillus, Ureaplasma urealyticum).
    • Lactobacilli were common in voided urine of both patients and controls.
    • Chlamydia trachomatis and Herpes simplex were detected in one patient.
    • Urethral cytology was normal in all patients.
    • Cystoscopy revealed trigonitis in 26 patients.
    • Bladder biopsies showed squamous metaplasia (15 patients) and lymphocytic infiltration (29 patients).

    Conclusions:

    • The study suggests that recurrent urethral syndrome is unlikely to be primarily caused by bladder infections in most cases.
    • Findings support an inflammatory etiology, evidenced by cystoscopic and histological changes.
    • Further research is needed to fully elucidate the pathogenesis of this syndrome.
