Summary

Background:

  • Bariatric surgery candidates often experience foot pain, depression, and elevated plantar pressures.
  • Limited research exists on the interplay of these factors before and after bariatric surgery.

Purpose of the Study:

  • To investigate mechanical and non-mechanical factors associated with foot pain severity in bariatric surgery candidates.
  • To examine changes in these factors and their relationship with foot pain pre- and post-surgery.

Main Methods:

  • Assessed 45 bariatric surgery candidates (29 surgery, 16 controls) at baseline and 6 months.
  • Measured foot pain (Manchester-Oxford Foot Questionnaire), depressive symptoms, body mass index (BMI), plantar pressures, foot posture, and hindfoot range of motion.
  • Used multivariable linear regression and multilevel repeated models to analyze associations.

Main Results:

  • Bariatric surgery led to significant reductions in foot pain (–35.7 points) and depressive symptoms (–5.9 points).
  • Depressive symptoms were associated with foot pain at baseline (β=0.7) and post-surgery (β=1.2).
  • No significant changes in foot posture or hindfoot range of motion were observed; changes in plantar pressures did not correlate with pain reduction.

Conclusions:

  • Foot pain in bariatric surgery candidates is linked to depressive symptoms.
  • Post-surgery improvements in foot pain correlate with improved mental health, independent of changes in foot mechanics.
  • Non-local factors, such as mental health, may significantly influence foot pain in this population.
