    Background:

    • Endurance training enhances functional capacity (VO2 max) and cardiovascular health.
    • Hypertension, particularly severe forms, poses risks for cardiovascular complications.
    • The impact of exercise on myocardial structure in hypertension is not fully understood.

    Purpose of the Study:

    • To investigate the effects of endurance training on functional capacity, blood pressure, and myocardial capillary density in normotensive and hypertensive rats.
    • To determine if exercise training influences cardiovascular adaptations differently based on hypertension severity.

    Main Methods:

    • Male Sprague-Dawley rats were subjected to sham, moderate hypertension (MH), or severe hypertension (SH) via Goldblatt's two-kidney one-clip method.
    • Trained (T) rats exercised on a treadmill (50-70% VO2 max) for 8-12 weeks; controls were non-trained (NT).
    • Measurements included VO2 max, cytochrome-c oxidase activity, blood pressure, heart weight, and myocardial capillary density.

    Main Results:

    • Training significantly increased VO2 max (12-15%) and muscle cytochrome-c oxidase activity (33-78%) in all groups.
    • Severe hypertension (SH) trained rats showed increased resting blood pressure (20-30 mmHg) and significantly reduced subepicardial and subendocardial capillary density.
    • Normotensive and moderately hypertensive trained rats showed no significant increase in resting blood pressure, with only normotensive trained rats exhibiting increased heart weight.

    Conclusions:

    • Moderate endurance training improves functional capacity in hypertensive rats.
    • However, exercise training appears detrimental in severe hypertension, exacerbating blood pressure and reducing myocardial capillary supply.
    • These findings suggest caution regarding exercise intensity and prescription in patients with severe hypertension.
