    Background:

    • Lithium is a common treatment for bipolar disorder.
    • Lithium use is associated with nephrogenic diabetes insipidus and hypothyroidism.
    • Adenylate cyclase inhibition has been proposed as a common mechanism for both lithium-induced effects.

    Purpose of the Study:

    • To investigate the relationship between lithium-induced hypothyroidism and impaired urine concentrating ability.
    • To determine if adenylate cyclase inhibition is the primary mechanism for both conditions.

    Main Methods:

    • Serum thyroxine levels and urine concentrating ability (Umax) were measured in 85 patients receiving lithium.
    • Response to desmopressin (DDAVP) was assessed.
    • Patients were monitored for the development of hypothyroidism.

    Main Results:

    • Eight patients developed hypothyroidism during lithium treatment.
    • Impaired Umax was observed in both euthyroid and hypothyroid patients.
    • Some hypothyroid patients maintained normal urine concentrating ability.

    Conclusions:

    • Lithium-induced hypothyroidism and impaired urine concentrating ability likely involve different dominant mechanisms.
    • The proposed common mechanism of adenylate cyclase inhibition may not fully explain both conditions.
