Summary

A new upper gastrointestinal (GI) capsule endoscope shows excellent views of the upper GI tract. This nurse-led capsule endoscopy is feasible and well-tolerated by patients, offering a promising alternative to traditional gastroscopy.

Purpose of the Study:

  • To assess the feasibility and performance of a novel upper GI capsule endoscope.
  • To evaluate a nurse-led protocol for upper GI capsule endoscopy.
  • To determine capsule transit times, visualization quality, and patient tolerance.

Main Methods:

  • Prospective cohort study of 50 patients who declined OGD but accepted upper GI capsule endoscopy.
  • Patients ingested a capsule with water and simethicone, followed by positional changes to guide the capsule.
  • Evaluated capsule transit, video reading time, mucosal visualization, pathology detection, and patient tolerance.

Main Results:

  • Excellent visualization of upper GI anatomical landmarks was achieved (mean scores 3.8-4.8/5).
  • The capsule reached the second part of the duodenum (D2) in 64% of patients.
  • Quickview video analysis significantly reduced reading time (20 min vs. 48 min) without missing pathology; procedural tolerance was excellent.

Conclusions:

  • The upper GI capsule endoscope provides excellent views of the upper GI tract.
  • A nurse-led protocol is feasible and effective for upper GI capsule endoscopy.
  • Further studies comparing diagnostic accuracy with OGD are warranted.
