    Background:

    • Upper gastrointestinal bleeding (UGIB) is a common emergency in the elderly.
    • Esophageal submucosal hematoma is a rare cause of dysphagia and hematemesis.
    • Cirrhosis is a frequent comorbidity associated with UGIB in older adults.

    Observation:

    • A 71-year-old woman presented with dysphagia and hematemesis.
    • Endoscopic examination revealed an esophageal submucosal hematoma.
    • Post mortem examination identified an aorto-esophageal fistula as the underlying cause.

    Findings:

    • The patient's hematemesis was attributed to a previously undiagnosed aorto-esophageal fistula.
    • The differential diagnosis for UGIB in elderly patients without cirrhosis was explored.
    • Literature review on aorto-esophageal fistulas was conducted.

    Implications:

    • This case underscores the need to consider rare vascular anomalies in the differential diagnosis of UGIB in the elderly.
    • Prompt recognition and management of aorto-esophageal fistulas are crucial for patient survival.
    • Further research into the diagnostic challenges and treatment strategies for aorto-esophageal fistulas is warranted.
