Outer plexiform layer (OPL) elevations in Behcet's disease patients correlate with prior posterior ocular attacks. These OPL bumps may serve as a clinical marker for managing Behcet's uveitis.

Background:

  • Behcet's disease is an autoimmune disorder that can affect the eyes.
  • Abnormalities in the outer plexiform layer (OPL), such as focal bumps, are common in Behcet's disease.
  • These OPL bumps can lead to compression of the inner nuclear layer.

Purpose of the Study:

  • To investigate the clinical relevance of outer plexiform layer (OPL) elevations in patients with Behcet's disease.
  • To determine if OPL elevations are associated with specific clinical parameters and disease history.

Main Methods:

  • Spectral-domain optical coherence tomography (OCT) was used to image 31 patients (59 eyes) with Behcet's disease in remission.
  • The number of OPL elevations was counted.

  • Relationships between OPL bumps and visual acuity, retinal thickness, choroidal thickness, disease duration, prior ocular attacks, and photoreceptor layer status (external limiting membrane [ELM] and ellipsoid zone [EZ]) were analyzed.

    • Eyes with more severe ellipsoid zone (EZ) or external limiting membrane (ELM) disruptions showed poorer visual acuity, more ocular attacks, and thinner retinas.
    • The status of the EZ and ELM lines significantly correlated with the number of OPL elevations.
    • Eyes with OPL elevations exhibited poorer visual acuity, longer disease duration, more ocular attacks, and thinner retinas compared to those without OPL elevations.
    • A strong correlation was found between the number of OPL elevations and the number of ocular attacks in eyes with preserved photoreceptor layers (R = 0.720, P < 0.0001).

    Conclusions:

    • The number of OPL elevations is associated with the history of prior ocular attacks in eyes with preserved photoreceptor layers.
    • OPL elevations may serve as a valuable biomarker indicating previous posterior ocular attacks in Behcet's disease.
    • Identifying OPL elevations can aid in the management strategy for Behcet's uveitis.
