Summary

Subclinical hypothyroidism (SCH) is common in men with erectile dysfunction (ED) and may contribute to its development. However, SCH severity does not correlate with ED severity, suggesting thyroid screening for men with ED.

  • Erectile dysfunction (ED) is a prevalent condition in males.
  • Hypothyroidism has been linked to ED, but the association with subclinical hypothyroidism (SCH) is less understood.

Purpose of the Study:

  • To investigate the prevalence and association of subclinical hypothyroidism (SCH) with erectile dysfunction (ED).
  • To compare hormonal profiles and erectile function between ED patients with and without SCH.

Main Methods:

  • Consecutive recruitment of ED patients from Urology Outpatients Clinic and controls from Health Management Center.
  • Measurement of serum thyroid hormones, sexual hormones, and evaluation using the International Index of Erectile Function (IIEF-5) questionnaire.
  • Statistical analysis of subjects with normal sexual hormone levels.

Main Results:

  • The study included 109 ED patients and 32 controls, with SCH found in 29.36% of ED males.
  • ED patients with SCH exhibited significantly lower IIEF-5 scores compared to euthyroid controls.
  • Elevated TSH and prolactin, with lower free thyroxine, were observed in ED patients with SCH compared to euthyroid controls.

Conclusions:

  • Subclinical hypothyroidism (SCH) is prevalent in men with erectile dysfunction (ED) and may be associated with its presence.
  • The severity of ED is not directly related to the presence or severity of SCH.
  • Screening for thyroid dysfunction is recommended for men presenting with ED.
