    Background:

    • Hamstring injuries are a common issue in Gaelic games.
    • Prospective research on hamstring flexibility as a risk factor is limited in this population.

    Purpose of the Study:

    • To investigate hamstring flexibility using the modified active knee extension (AKE) test and previous injury as risk factors for hamstring injuries in Gaelic players.
    • To establish population-specific AKE cutoff points for injury prediction.

    Main Methods:

    • A prospective cohort study involving 570 adolescent and collegiate Gaelic athletes.
    • Modified AKE tests were conducted preseason, with hamstring injuries tracked over one season.
    • Logistic regression and receiver operating characteristic curves were used to analyze data.

    Main Results:

    • Mean AKE flexibility was 64.2° (dominant leg) and 64.1° (nondominant leg).
    • A cutoff point of <65° on the nondominant leg was identified, but with 0% sensitivity.
    • The AKE test on the nondominant leg was a significant predictor (OR=1.03) but did not improve prediction over chance.

    Conclusions:

    • Preseason hamstring flexibility (AKE test) and prior injury history were insufficient to predict hamstring strain risk in Gaelic games.
    • The modified AKE test demonstrated inadequate sensitivity for identifying at-risk athletes in this cohort.
