Insights
Female sex, active disease, extreme body weight, and older age at diagnosis are linked to lower quality of life (QoL) in Asian pediatric patients with inflammatory bowel disease (IBD). Regular QoL assessments are crucial for understanding these patients.
Area of Science:
- Pediatric Gastroenterology
- Inflammatory Bowel Disease Research
- Quality of Life Studies
Background:
- Inflammatory bowel disease (IBD) is a chronic condition significantly impacting patients' quality of life (QoL).
- Understanding factors influencing QoL in pediatric IBD patients is crucial for effective management.
Purpose of the Study:
- To evaluate the QoL in Asian pediatric patients diagnosed with IBD.
- To identify clinical factors associated with QoL variations in this demographic.
Main Methods:
- A cohort of 208 children and adolescents (ages 9-18) with IBD was recruited from seven hospitals.
- Patients completed the IMPACT-III questionnaire to assess QoL.
- Clinical data were collected and analyzed for correlations with QoL scores.
Main Results:
- Female sex, active disease status, extreme body weight z-scores, and older age at diagnosis were significantly associated with poorer QoL.
- No significant QoL differences were observed based on the Paris classification.
- Specific statistical associations: Female sex (P=0.0347) and active disease (P=0.0006) showed significant negative impacts on QoL.
Conclusions:
- Clinical factors like disease activity and patient demographics influence QoL in pediatric IBD.
- Relying solely on clinical factors is insufficient for a comprehensive understanding of patient QoL.
- Direct, regular measurement of QoL is essential for better patient care and management in pediatric IBD.
Objectives:
Inflammatory bowel disease (IBD) is a chronic lifelong condition and is related to poor quality of life (QoL). The aim of this study was to evaluate the QoL of Asian pediatric patients with IBD and to determine the clinical factors that can influence QoL.
Methods:
Children and adolescents aged 9 to 18 years diagnosed with IBD were enrolled from 7 hospitals. The patients completed the IMPACT-III questionnaire, and clinical data were collected. The results of the questionnaire and the correlation with clinical data were analyzed.
Results:
A total of 208 patients (Crohn disease: n = 166; ulcerative colitis: n = 42) were enrolled. There was no definite QoL difference according to the Paris classification. Female sex (-5.92 ± 2.97, P = 0.0347) and active disease status (-10.79 ± 3.11, P = 0.0006) were significantly associated with poor QoL. Extreme body weight z score and older age at diagnosis were also associated with worse QoL.
Conclusions:
Various clinical factors may affect the QoL in patients with IBD, but determining the overall QoL of patients using only these clinical factors is difficult. Therefore, regular direct measurements of QoL are necessary to better understand patients with IBD.

Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
Crohn's disease
Crohn's disease is a chronic, systemic inflammatory bowel disease (IBD) that predominantly affects the gastrointestinal tract. It is marked by...
Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...

Diagnostic studies
A colonoscopy is the definitive screening test, distinguishing ulcerative colitis from other colon diseases with similar symptoms. During a colonoscopy test, inflamed mucosa with exudate ulcerations can be observed, and biopsies are taken to determine the histologic characteristics of the...
