Summary

Dermatofibrosarcoma protuberans (DFSP) is a rare breast cancer. Complete surgical resection with wide margins and long-term follow-up are crucial for preventing recurrence of this skin neoplasm.

Area of Science:

  • Oncology
  • Dermatology
  • Surgical Pathology

Background:

  • Dermatofibrosarcoma protuberans (DFSP) is a rare, locally aggressive skin neoplasm.
  • DFSP typically presents on the trunk and extremities, with breast involvement being extremely rare.
  • Standard treatment involves wide local surgical excision (2-3 cm margins).

Observation:

  • A 49-year-old female presented with a progressive, benign-appearing breast lump.
  • Initial mammography and physical examination were non-diagnostic for malignancy.
  • Microscopic and immunohistochemical analysis confirmed DFSP with a positive deep margin after initial resection.

Findings:

  • Complete resection of the positive deep margin was performed following multidisciplinary discussion.
  • The patient remained disease-free for 3 years post-re-excision, with no local or distant recurrence.

  • This case highlights the importance of adequate surgical margins in managing DFSP.
  • Implications:

    • Emphasizes the necessity of thorough histopathological evaluation for suspected breast neoplasms.
    • Underscores the critical role of wide surgical margins in preventing DFSP recurrence.
    • Recommends long-term surveillance including physical examination and ultrasound for early detection of DFSP recurrence in the breast.
