Background:

Cutaneous lupus erythematosus (CLE) encompasses various skin manifestations of lupus erythematosus (LE), a chronic autoimmune connective tissue disease.
Lupus profundus (LP) is a rare subtype of CLE, accounting for only 5% of cases, often presenting as subcutaneous lesions.

Observation:

A 40-year-old female presented with biopsy-proven lupus profundus, characterized by high-grade fever and multiple ulcerated lesions on her thigh, gluteus, and arm.
Lesions exhibited erythematous bases with edematous, necrotizing centers and purulent discharge, accompanied by systemic symptoms like oral ulcers, joint pain, and photosensitivity.

Findings:

The case highlights ulcerated lupus profundus as a rare differential diagnosis for painful, indurated subcutaneous plaques.
Despite unremarkable autoimmune assays, the patient's presentation and histopathology confirmed lupus profundus, potentially preceding or co-occurring with systemic lupus erythematosus (SLE).

Implications:

Early histopathological diagnosis and aggressive treatment of lupus profundus are essential for managing physical morbidity.
Prompt intervention may prevent the progression of cutaneous lupus erythematosus to systemic lupus erythematosus (SLE).
