Electrophilic Aromatic Substitution: Fluorination and Iodination of Benzene

Bromination and chlorination of aromatic rings by electrophilic aromatic substitution reactions are easily achieved, but fluorination and iodination are difficult to achieve. Fluorine is so reactive that its reaction with benzene is difficult to control, resulting in poor yields of monofluoroaromatic products. To address this, Selectfluor reagent is used as a fluorine source in which a fluorine atom is bonded to a positively charged nitrogen.

Background:

Delayed drug reactions are a significant clinical challenge.
Iodinated contrast media (ICM) are widely used in medical imaging.
Underrecognition of ICM as a cause of delayed reactions leads to continued patient exposure.

Observation:

A case of delayed cutaneous reaction to iodinated contrast is presented.
The reaction manifested as a rash occurring after contrast administration.

Findings:

Delayed hypersensitivity reactions to ICM are an underrecognized phenomenon.
Literature review highlights the epidemiology and management strategies for these reactions.

Implications:

Increased awareness among healthcare providers is essential.
Improved diagnostic approaches are needed to identify ICM-induced delayed reactions.
Preventing readministration of ICM can mitigate patient morbidity.
