    Background:

    • Primary hypothyroidism is a common endocrine disorder.
    • Autonomous thyrotoxicosis, or hyperthyroidism, is typically associated with conditions like Graves' disease or toxic nodules.
    • The progression from hypothyroidism to hyperthyroidism is an unusual clinical presentation.

    Observation:

    • Two patients presented with autonomous thyrotoxicosis after a diagnosis of primary hypothyroidism.
    • In one patient, TSH receptor antibodies indicated Graves' disease, yet thyroid biopsy showed lymphocytic thyroiditis.
    • A review of 21 similar cases from the literature was conducted.

    Findings:

    • The sequence of developing autonomous thyrotoxicosis after primary hypothyroidism is more common than previously thought.
    • Thyroid autoimmunity can manifest in complex and evolving patterns.
    • TSH receptor antibodies and histological findings may not always align in these cases.

    Implications:

    • Clinicians should consider the possibility of developing hyperthyroidism in patients with a history of hypothyroidism.
