Trichilemmal carcinoma, a rare hair follicle neoplasm, presents variably and requires histologic diagnosis. This case highlights an aggressive form of this rare cancer located in the auricle.

Background:

Trichilemmal carcinoma is a rare adnexal neoplasm.
It originates from the outer root sheath of hair follicles.
Its clinical presentation is often variable.

Observation:

The patient presented with an aggressive trichilemmal carcinoma.
The tumor was located in the auricle (ear).
Clinical appearance mimicked other common skin cancers.

Findings:

Histologic examination confirmed trichilemmal carcinoma.
The diagnosis required differentiating it from basal cell and squamous cell carcinomas.
The auricle is an unusual site for this aggressive neoplasm.

Implications:

Accurate histologic diagnosis is crucial for managing trichilemmal carcinoma.
Awareness of its variable presentation is important for clinicians.
This case underscores the potential for aggressive behavior and specific site involvement.
