    Summary

    A small cell carcinoma initially presenting in a lymph node was later found to be a primary neuroectodermal tumor of the testis. This case highlights a rare presentation of testicular cancer.

    • Small cell carcinoma (SCC) is an aggressive cancer that can originate in various organs.
    • Metastasis of unknown primary SCC often presents diagnostic challenges.

    • A 51-year-old male with a history of inguinal lymph node metastasis from SCC of unknown origin.
    • Development of a testicular swelling three years after initial diagnosis.
    • Histopathological examination revealed a proliferation of small tumor cells occupying the testicular parenchyma.

    Findings:

    • Tumor cells exhibited features of neuroectodermal differentiation.
    • Electron microscopy showed tumor cells with dense-core vesicles and dendrite-like prolongations.
