Accidental surgical incision into the tumour (ASIT) during robotic partial nephrectomy (RPN) is common but does not negatively impact cancer recurrence-free survival. Further research should explore minimizing ASIT during RPN for renal cell carcinoma.
Area of Science:
- Urology
- Oncology
- Surgical Innovation
Background:
- Robotic partial nephrectomy (RPN) is a standard treatment for localized renal cell carcinoma (RCC).
- Accidental surgical incision into the tumour (ASIT) can occur during RPN, raising concerns about oncological outcomes.
- Understanding the impact of ASIT is crucial for refining surgical techniques and patient counseling.
Purpose of the Study:
- To evaluate the oncological outcomes in patients who experienced ASIT during RPN for malignant renal tumours.
- To identify predictive factors associated with ASIT during RPN.
Main Methods:
- Retrospective review of a prospectively maintained database of patients undergoing RPN for localized RCC (June 2010 - July 2016).
- Cohort stratified into groups with and without ASIT.
- Comparison of perioperative data, recurrence-free survival (Kaplan-Meier, log-rank test), and logistic regression for ASIT predictors.
Main Results:
- 32% of 234 patients experienced ASIT.
- ASIT group had significantly higher estimated blood loss (EBL).
- Tumour size, RENAL score, EBL, and surgeon experience were predictive of ASIT. No significant difference in recurrence-free survival between groups.
Conclusions:
- ASIT during RPN for renal tumours is a frequent occurrence.
- ASIT does not appear to compromise oncological outcomes or recurrence-free survival.
- Surgeon experience and tumour characteristics are associated with ASIT incidence.
