    Area of Science:

    • Neurosurgery
    • Neuroradiology
    • Neurology

    Background:

    • Syringomyelia is a condition characterized by a fluid-filled cyst within the spinal cord.
    • Surgical intervention is a primary treatment modality for syringomyelia.
    • Accurate diagnosis and pre-operative planning are essential for successful surgical outcomes.

    Purpose of the Study:

    • To evaluate the role of radiological examinations in the diagnosis and surgical management of syringomyelia.
    • To correlate radiological findings with surgical confirmation of associated conditions.
    • To emphasize the importance of early neuroradiological diagnosis for prompt surgical treatment.

    Main Methods:

    • Retrospective analysis of 20 patients surgically treated for syringomyelia between 1973-1975.
    • Detailed review of plain radiograms, ascending myelography, and central pneumoencephalography findings.
    • Correlation of pre-operative radiological diagnoses with intra-operative findings.

    Main Results:

    • Plain radiography revealed cervicothoracic scoliosis (16 cases), cervical vertebral canal dilatation (11 cases), and atlanto-occipital malformations (6 cases).
    • Ascending myelography was performed in 17 patients; contrast passage was impeded in 3 due to cervical spinal cord dilatation.
    • Arnold-Chiari syndrome was suspected radiologically in 9 of 14 myelographed cases and confirmed surgically. One case showed a syrinx-ventricle communication via pneumoencephalography.

    Conclusions:

    • Comprehensive radiological assessment is indispensable for confirming syringomyelia diagnosis.
    • Imaging findings guide the selection of appropriate surgical methods and indicate associated pathologies.
    • Early and detailed neuroradiological diagnosis facilitates timely surgical intervention, improving patient outcomes.
