  • Initial clinical and imaging studies suggested a mesenteric cyst.

Findings:

  • Laparoscopic excision of the suspected mesenteric cyst was performed uneventfully.
  • Postoperative histopathology confirmed the mass to be an infarcted accessory spleen.

Implications:

  • This case underscores the importance of considering accessory spleen infarction in the differential diagnosis of abdominal masses, even when presenting atypically.
  • Accurate preoperative diagnosis and surgical management are crucial for resolving symptoms and preventing complications associated with symptomatic accessory spleens.
